Interim HealthCare of Richmond is adding a Registered Nurse Case Manager to perform Start of Care and Supervisory Visits for our strong and growing Personal Care client base. This position can be part-time with a weekly schedule, or full-time averaging 4-5 days per week. We will work with the successful candidate to simply set the schedule that works for you, while covering our RN needs. Requirements

  • Graduate of an accredited nursing program and active RN license in Virginia
  • Minimum of (2) Two years of RN nursing experience, ideally in home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills and compassion

Responsibilities

  • Develop and update individualized Plans of Care (POC) based on client needs, physician orders, and agency policies.
  • Ensure timely reassessments and supervisory visits per state and agency requirements.
  • Serve as a liaison among physicians, clients, families, and other interdisciplinary team members.
  • Participate in quality assurance activities, chart audits, and staff meetings.
  • Coach family members on the patient’s plan of care, medication and home safety
  • Communicate changes in condition, medication, or care needs promptly to the appropriate parties.
  • Maintain accurate and timely documentation in the electronic health record (EHR) system.
