Requirements

  • Graduation from an accredited School of Nursing; License and current registration as a Registered Professional Nurse in New York State.
  • Strong Computer, Verbal and Written Communication Skills
  • Ability to work independently, multiple assignments, prioritize workload.
  • Valid NYS driver’s license and ability to drive in all types of weather.
  • Standing, sitting, bending, walking and driving (frequently entering and exiting vehicle throughout the day) are standard postures.
  • Frequently uses a laptop computer to enter orders and obtain product information.

Responsibilities

  • The Home Care RN Case Manager provides and coordinates care to patients in the home setting utilizing the nursing process of assessment, planning, implementation and evaluation of safe and therapeutic care.
  • Ensures quality nursing care is rendered to all.
  • Utilizes the Nursing Process, plans, directs and supervises the care of patients in addition to being involved in the provision of direct care of patients and families.
  • Develops a written plan of care that guides nursing intervention and facilitates desired outcomes.
  • Implements interventions based on the plan of care in accordance with client and caregiver level of skill and knowledge.
  • Evaluates client/ family responses to interventions and changes the plan as required.
  • Document care provided and client/ family responses consistent to agency policy and procedure.
  • Follows agency policies and procedures.
  • Communicates with physician and other disciplines regarding client status. Coordinates services as needed.
  • Utilizes community resources in the development of plan of care.
  • Supervises ancillary personnel (Home Health Aide, Shift Professional Nurse) assigned to client care.
  • Attends educational programs to maintain current standards of practice.
  • Maintains agency’s expected productivity standards.
  • Responsible for employee injury prevention, follows safety rules, and addresses safety issues with supervisor.
  • All other duties as assigned by supervisor.
