About The Position

The Supervised Independent Living Program (SILP) provides a transitional living environment for older youth (18-20 years old) who are ready to move towards independent living but require a supportive environment to develop the necessary skills. The Case Manager is the most senior level of direct care staff within the Jewish Board’s children’s community residential programs. This role provides support and counseling aimed at stabilization, the incorporation of independent living skills, and successful discharge into the community.

Requirements

  • Bachelor’s degree required.
  • Excellent resident engagement skills.
  • Strong verbal and written communication skills.
  • Attention to detail.
  • Ability to work independently as well as with a team.
  • Willingness to work with adolescents/young adults living with the impact of trauma and experience mental health challenges.
  • Compassion and respect for vulnerable individuals.
  • Patience and understanding for the sometimes-slow process of stabilization and recovery.
  • Familiarity with working in Windows environment.
  • Ability and willingness to learn new systems and application software.

Nice To Haves

  • Master’s degree in Human Services or related field preferred.
  • Familiarity with Connections (ACS record).
  • Familiarity with telehealth.

Responsibilities

  • Carries a caseload of residents and provides services within program regulations and agency mission.
  • Evaluates and assesses independent living skills and supports clients in achieving individual goals.
  • Advocates for residents with other service providers.
  • Links residents to community-based treatment and collaborates with other service providers.
  • Practices person-centered and collaborative service planning with clients, ensuring plans are completed and signed by residents.
  • Interprets and explains agency policies and procedures to residents.
  • May run groups under supervision.
  • Supports residents toward medication management independence through person-centered interventions.
  • Accompanies residents to appointments as needed.
  • Participates in intake, assessment, discharge, and follow-up conferences.
  • Participates in regular staff meetings and trainings, potentially traveling to offsite locations.
  • Participates in planning and execution of special and holiday events.
  • Files incident reports as appropriate.
  • Completes regular and timely documentation of all resident interactions, including progress and informational notes.
  • Ensures progress notes meet monthly billing documentation requirements, if applicable.
