About The Position

The Case Manager is the primary point of contact and liaison for internal/external resident care and services. The Case Manager has the responsibility to provide case management/care coordination and advocacy by carrying a caseload and assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess the individual’s needs on an on-going basis.

Requirements

  • Bachelor's degree from an accredited program in an area of human services.
  • A minimum of one year experience (paid or unpaid) in a human service field.
  • This experience must involve direct contact with the individual receiving services
  • PA driver's license and criminal background check required.

Responsibilities

  • Acts as the primary liaison between residents and community resources, including federal, state, county agencies, and private companies.
  • Ensure that all entitlements are received and current and manages the oversight of insurance coverage
  • Promotes and maintains safety, cleanliness, and regulatory compliance of the milieu using person first language, safe crisis intervention, and trauma informed care while treating residents with dignity and respect.
  • Ensure and maintain regulatory compliance of the individual’s record.
  • Establishes, maintains and audits resident files to include: admission paperwork, quality progress notes, medical evaluations, benefit paperwork, etc.
  • Provides one-to-one support to assigned residents in completing Activities of Daily Living (ADL’s) in accordance with each resident’s support plan; these may include assistance with bathing, toileting, dressing, and other personal care activities.
  • Provides one-to-one support to assigned residents in completing Instrumental Activities of Daily Living in accordance with each resident’s support plan; these may include cleaning, laundry, shopping, providing transportation, support with recreational, educational or vocational pursuits, using the telephone, etc.
  • Maintains contact with families, teams, or designated allies of residents; including emergent issues, progress updates (if approved by individual), and involvement for recovery.
  • Develops comprehensive plan for each assigned resident, which includes an overview of medical, psychiatric, and daily living needs. Updates annually or as appropriate.
  • Coordinates, consults and collaborates with the resident’s team, and participates regularly in providing input into the clinical recovery plans for all residents receiving therapeutic services.
  • Maintains ongoing contact with the resident’s treatment team members; report changes, concerns, complaints, and progress to enhance resident’s overall wellness.
  • Assists in arranging medical, psychiatric, legal, vocational, and other necessary appointments and accompanies residents as needed.
  • Assesses and monitors self-care and life skills of assigned residents and provides education and encouragement to achieve the highest level of independence possible.
  • Assists residents in identifying and planning for achievement of short and long-term goals, including: vocational, educational, recreational, and future planning.
  • Assists residents with the management of any current or ongoing legal issues.
  • Manages resident hospital admissions for those requiring an acute increase in care (medical, voluntary, or involuntary).
  • Coordinates discharge planning for resident, including obtaining signed releases of information, referrals, continuing care planning and the gathering and organization of all relevant paperwork.

  • Adaptability to changes within the workplace
  • Attends mandatory staff meetings and training sessions during orientation and as required thereafter
  • Ability to provide verbal and written communication
  • Computer literate with required technology, software and applications
  • Maintain respectful and professional relationships with company employees, residents, program members, and outside vendors
  • Proactive collaboration while identifying additional needs, tasks, and priorities within the organization
  • Maintaining the confidentiality and security of all New Vitae staff and consumer data and information
  • Accountability of personal responsibilities and essential functions of the position
  • Maintain accountability for companywide personnel with regards to policies/procedures, regulatory and stakeholder requirements, best practices and identified corporate values/goals
  • Has an understanding of the following: mental health, intellectual disabilities, substance use, brain injury, and chronic medical conditions
  • Monitors interactions of residents and provide support/intervention, when necessary, to ensure the physical and emotional safety of all residents.
