Case Manager, Rehabilitation PRN. Your experience matters! Here, you’re not just valued as an employee, but as a person. Temple Rehabilitation Hospital is a 36-bed inpatient acute rehabilitation facility located at 23621 SE H.K. Dodgen Loop in Temple, Texas. It’s designed to help individuals recover from serious injuries or illnesses that have significantly impacted their ability to function independently. Specialized Rehabilitation Programs: Tailored care for patients recovering from stroke, brain injury, spinal cord injury, amputation, orthopedic trauma, and other neurological or complex conditions. Intensive Therapy Model: Patients receive at least three hours of physical, occupational, and/or speech therapy per day, five days a week, under the supervision of a rehabilitation physician. Requirements

• Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Effective oral and written communication in English.
• Proficiency in Microsoft Office applications required.

Nice To Haves

• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred.
• Additional languages preferred.

Responsibilities

• Coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting.
• Serving as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.
• Completing departmental orientation, as well as initial and annual competencies.
• Collaborating with interdisciplinary team members to identify barriers to care or discharge and develop appropriate solutions.
• Documenting patient care activities in alignment with workflow timelines, including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
• Scheduling family conferences and communicating progress and discharge planning with caregivers after team conferences and as needed.
• Coordinating weekly interdisciplinary team conferences to monitor treatment goals and outcomes.
• Reviewing assigned Case Mix Group (CMG) and assisting the team in identifying actively treated comorbid conditions; communicating findings to the Health Information Management (HIM) team.
• Performing other duties as assigned.
