About The Position

Case Manager - PHAP Peer Assistance Services, Inc. is a community-based, 501(c)(3), non-for-profit Colorado corporation. Our mission, to lead with prevention and intervention for substance use and mental health concerns, guides all agency programming. PAS implements two statewide programs, Screening Brief Intervention, Referral to treatment (SBIRT), and the Peer Health Assistance Programs (PHAP) The statewide Peer Health Assistance Programs provide assessment, referral, long-term case management, and monitoring for health and mental health professionals with physical, emotional, psychiatric, psychological, and/or substance use conditions affecting their practice. Programming includes statewide outreach, and communications with multiple professions. About the Case Manager Position This is a hands-on, full-time position with day-to-day responsibilities that will include ensuring program participants are able to practice their profession with reasonable skill and safety, while complying with regulatory requirements. You will also collaborate with licensed evaluators to develop a monitoring contract outlining treatment and community resources to optimize program participants' physical and mental health and wellbeing. You will need to maintain regular communication with program participants, treatment providers, and workplace supervisors to measure compliance with the monitoring contract. You may also have to adjust monitoring contract requirements based on compliance measurements while keeping accurate records of contract monitoring throughout the program participant's involvement in the Peer Health Assistance Program. Your goal will be to help program participants achieve optimal health and well-being while also meeting regulatory requirements.

Requirements

  • Bachelor’s degree in a health or behavioral health related field is required
  • Two or more years of experience in case management is required
  • Excellent verbal and written communication skills.
  • Attention to detail and organization skills.
  • Deliver high-quality services to program participants and contracts.
  • Critical thinking

Nice To Haves

  • Experience in community mental health settings such as forensic or competency-related services is preferred
  • Strong skills in structured monitoring, compliance tracking, and participant accountability

Responsibilities

  • Provides comprehensive case management for Peer Health Assistance Program participants
  • Determining monitoring contract requirements in collaboration with clinical evaluators.
  • Providing referrals to community resources, and all levels of mental health or substance use treatment.
  • Communicating with providers to ensure comprehensive care.
  • Monitoring compliance through correspondence with treatment providers and employers, verification of treatment and support group attendance, and review of drug testing results.
  • Reviewing all incoming reports, screening tools, and correspondence for accuracy, and compliance; manages a high volume of documentation and identifies concerns requiring follow-up or clinical escalation.
  • Approving potential job opportunities to ensure alignment with program requirements and safety to practice.
  • Develop practice agreements that outline employment modifications and reporting expectations.
  • Modifying monitoring contracts based on documented compliance, recommendations from providers, return to substance use, or a recurrence of mental health symptoms.
  • Conducting regular check-ins with each participant on a case load.
  • Preparing discharge requirements and documentation.
  • Maintains regular communication with regulatory boards/programs regarding Participant compliance, status updates, and program requirements.
  • Presents complex cases during interdisciplinary case consultation, documents internal case staffing discussions, clinical recommendations, and compliance considerations, and ensures timely follow through on agreed upon action steps.
  • Prepare quarterly compliance reports.
  • Participates in program outreach by staffing exhibit tables, providing presentations, and attending events for partner organizations.
  • Establishes and manages relationships with key partners, e.g., treatment and healthcare providers, employers, supervisors, and practice monitors.
  • Attends staff meetings including case consultation, office hours, case review, required training, and all agency meetings.
  • Assists with additional program activities as requested.
