The Community Partners Case Manager will report directly to the Community Partners Supervisor and provide services for Community Partners for Safe Families. This role involves working independently in the field, providing home-based and community-based services to families referred from various sources. The Case Manager must exercise professional judgment to assess needs, develop individualized service plans, manage crises, and make sound decisions in real-time. Responsibilities include conducting home visits, intakes, and assessments, coordinating supportive services, and maintaining an ongoing caseload. The role utilizes evidence-based, trauma-informed practices to support family stabilization and child safety. Case Managers engage new families, coordinate with service providers, manage their schedules, and prioritize needs to ensure consistent, high-quality support.
