The Case Manager provides comprehensive, client-centered services to pregnant and parenting individuals receiving outpatient substance use support. This role is responsible for assessment, treatment planning, care coordination, trauma-informed service delivery, progress monitoring, and discharge planning. Case Managers work collaboratively with a multidisciplinary team to support recovery, family stability, and healthy outcomes for parents and families. Case Managers are expected to meet the productivity standards outlined by their position in accordance with program and payer requirements.
