About The Position

IPMG is offering a sign-on bonus within the first 6 months of employment. IPMG is 100% Employee Owned and participates in an Employee Stock Ownership Plan (ESOP). This full-time, hybrid position requires travel to community-based meetings multiple times per week and working remotely from a home office setting. Applicants must live within the State of Indiana or within 10 miles of the state line due to in-person meeting requirements.

Requirements

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
A valid driver's license, car insurance, and reliable transportation
The ability to pass thorough background screens
Designated home office setting with computer, internet service, and smart phone

Nice To Haves

Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

Responsibilities

Provide information and education to help Individuals understand the Medicaid waiver process
Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
Help Individuals access services and supports they need to meet their goals
Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
