Reporting to the Social Services Supervisor, the Case Manager, Woodhull provides outreach and case management services to people experiencing homelessness, some with mental illness and/or substance use disorders, in order to assist the individual in the transition to permanent housing. This position has a 35-hour work week. This position requires completion of the ‘Master Agreement Health Clearance’ and attestation of compliance with New York State Department of Health requirements for hospital staff, with required documentation submitted prior to start date.
