About The Position

The Case Manager for Eagle Lake Children’s Center is responsible for assessing the needs of unaccompanied minor children in care, developing Individual Service Plans, screening for Human Trafficking concerns, facilitating the safe and timely release or discharge of children and youth, and documenting the provision of services in the child’s case file. The Case Manager will work primarily to help reunify unaccompanied minor children who are placed at the Eagle Lake Children’s Center by the Office of Refugee and Resettlement (ORR).

Requirements

  • Must be at least 21 years old.
  • Proficiency in Spanish/English (written and spoken language skills) required.
  • Intermediate proficiency in Microsoft Office products and Google tools.
  • Successfully pass a TB test annually.
  • Successfully pass a drug screen.
  • Be physically, mentally, and emotionally capable of performing assigned tasks and have the skills necessary to perform assigned task.
  • Pass a criminal history screen, including state, federal, and local child protection agency registries.
  • Ability to work independently and exercise a high level of confidentiality.
  • Affidavit for Applicants for Employment with a Licensed Operation or Registered Children’s Home.
  • Bachelor’s degree in a behavioral science, human services or social services field.
  • Driver’s License with clear record required.
  • Must be available and willing to travel in case of an emergency evacuation and as the business need dictates.
  • Must be available and willing to work nights, weekends and holidays as required to meet business needs.

  • ORR Case Management experienced, preferred.

Responsibilities

  • Oversees all aspects of the Unaccompanied Child’s (UC) case and provides release recommendation for safe and timely release.
  • Maintains a caseload of up to 8 clients per ORR ratio.
  • Maintain case file documentation in accordance with ORR requirements.
  • Maintains confidentiality when gathering, documenting, consulting or consolidating information necessary for the case management assessment and the Individual Service plan of care.
  • Maintain regular contact with each UC, UC’s family when possible, and establish service providers that will provide services to the child upon release.
  • Gathers historical and current data relative to completion of a comprehensive case management assessment including, but not limited to, school records, medical records, mental health treatment, legal status and court involvement, social and family strengths and needs, support systems, and specific cultural issues.
  • Presents case and case updates to federal stakeholders on a weekly basis.
  • Participates in treatment team meetings, staff in-service training and department meetings.
  • Completes Significant Incident Reports (SIR’s), per federal policy requirements.
  • Conducts internal chart reviews to monitor documentation compliance.
  • Other duties as assigned.
