About The Position

Under the general supervision of the Case Manager Supervisor, Case Managers are expected to perform the following principal duties and responsibilities. Conduct initial intake including but not limited to social service assessments, psychosocial, independent living plans, and referrals of incoming clients who are referred from the shelter system Conduct updates to psychosocial assessments and other evaluations as a basis for developing appropriate referrals, changes in treatment planning, and other interventions. Develop Individual Living Plan (ILP) with client participation. Identify benefits or entitlements that may be available. Connect clients to psychiatric services Connect clients to medical treatment & substance use treatment facilities when applicable Obtain identification, public benefits and other applicable income sources for clients Meet biweekly with clients to assess ongoing needs, encourage harm reduction, and update service plans Document all efforts in providing services, interactions, follow-up regarding resident involvement. Maintain up-to-date confidential client case records for assigned caseload. Complete daily tracking for ILP completion reports. Conduct work related functions with staff, clients and others, in a professional manner. Serve as a liaison to appropriate support service agencies and programs e.g., Health facilities; Income Maintenance Center, Board of Education, etc.) Participate in regular scheduled social service team meetings. Comply with AAPCI policies and procedures. Maintain professional growth and responsibilities. Perform other related duties as assigned

Requirements

  • Bachelor’s degree relevant to human services and at least one (1) year of proven experience in Human Service related field or;
  • An Associate Degree with at least two (2) – three (3) years of proven experience in a Human Service related field.
  • Must possess sensitivity, tact, and resourcefulness.
  • An eagerness to assist clients in a nonjudgmental, empathetic and compassionate manner.
  • Must demonstrate responsiveness to cultural differences and a commitment to the value of cultural competency.
  • Ability to develop and maintain appropriate boundaries.
  • Excellent organizational, time management and writing skills.

Nice To Haves

  • Bi-lingual preferred
  • Familiarity with CARES preferred.
  • Experience working with DHS and other service providers, Preferred.
  • Experience providing case management services to families with children referred from DHS.
