About The Position

This is a Full-Time Case Manager position. The Case Manager will conduct intake orientation with all patients, explaining rules, policies and procedures. They will also complete Case Management assessments and reports, develop working relationships with outside agencies, and evaluate patient needs and providing referrals as appropriate. The role involves acting as a liaison between referral sources and providing updates on patient progress in treatment, as well as assisting patients in obtaining necessary resources such as housing, food stamps, medical and benefit assistance.
