About The Position

Mountain Home is seeking a Case Manager (CM) to enhance the outcomes, experiences, and overall value for the families we serve. The CM will provide comprehensive case management services to young mothers, adolescents, and children, connecting them to essential medical, social, educational, vocational, and recreational services. Additionally, the CM will serve as an advocate, supporting families in building their independence and increasing their self-sufficiency within the community. This role is pivotal in ensuring that families receive the resources and support necessary to thrive. The ideal candidate has a passion and talent for helping others, communicating clearly and compassionately, and has the ability to connect and empathize with individuals, including diverse and marginalized populations. Communicate clearly, professionally, and with cultural sensitivity, fostering mutual respect and understanding. Ability to engage with all stakeholders and put people at ease—especially across lines of difference—reflects professionalism, empathy, and commitment to inclusive communication.
  • Strong organizational and time-management skills, including the ability to manage a caseload, track information accurately, meet documentation and billing timelines, and prioritize competing demands. Reliable in maintaining scheduled hours and completing required tasks. Plan work intentionally to stay within deadlines and focus on activities that best support client care and program needs.
