About The Position

Case Manager at Kingsboro Men’s Shelter MICA Program. This is a 143-bed facility located at 681 Clarkson Avenue Brooklyn NY 11203. The Case Manager renders direct casework and/or group work services to residents and is responsible for assessing, referring, and follow-up of social services.

Requirements

  • Baccalaureate Degree from an Accredited College or University in Human Services.
  • Experience preferably in program serving the homeless or similar population working with the homeless population.

Responsibilities

  • Performing a variety of casework duties, including meeting with residents and preparing psychosocial evaluations, including obtaining, analyzing and evaluating data and developing service plans.
  • Making periodic reports to the supervisor on the progress of each case.
  • Making the residents aware of rules and regulations of the shelter as well providing information regarding the services plan process.
  • Carrying a caseload and performing record keeping functions as required, case notes, census statistical reports.
  • Maintaining all records at a confidential level.
  • Referring residents to agencies and programs as necessary and establishing a rapport and professional relationship with these agencies.
  • Assisting residents in establishing relationships with outside services.
  • Arranging for transportation to appointments.
  • Preparing a facility workshop and meeting with clients, relative to goals of Social Services and needs of residents.
  • Working closely with other personnel assigned to residents.
  • Handling emergencies as they arise.
  • Arranging for medical services.
  • Attending staff meetings as requested by the Director of Social Services.
