About The Position
ProJeCt of Easton is hiring a full-time Case Manager to help individuals and families overcome challenges such as utility shutoffs, food insecurity, and stalled plans. The role involves connecting clients with emergency assistance, housing and utility support, adult and family literacy programs, youth services, and the food pantry. This position utilizes a strengths-based and trauma-informed approach, with a manageable caseload and direct supervision from the Director of Programs.
Requirements
- Associate degree in human services, social work, psychology, education, or a related field, or equivalent case management experience.
- Ability to follow through on plans, referrals, and files.
Nice To Haves
- Housing or utility assistance experience.
- Bilingual English/Spanish.
- Driver's license.
Responsibilities
- Assess client needs and develop service plans that are updated as life circumstances change.
- Assist clients in applying for rental, heating, and utility assistance to prevent denials due to missing paperwork.
- Facilitate client navigation of programs such as SNAP, Medicaid, LIHEAP, housing, employment, transportation, and healthcare, with internal handoffs within ProJeCt.
- Collaborate with landlords, utility companies, and partner organizations to resolve specific client issues.
- Provide support to adult learners, families, and assigned Out-of-School Youth.
- Write case notes on the same day of service and maintain funder files to be audit-ready.
