About The Position

The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to ensure appropriate use of hospital resources and determine whether patients are admitted to the appropriate level of care. The Case Manager assesses discharge needs and coordinates an appropriate discharge plan, working with the interdisciplinary team to support desired financial and quality outcomes. The role also maintains and enhances payor relationships.

Requirements

• Current BLS certification.
• Three to five years of inpatient experience; experience in an acute, IRF, or LTACH setting is preferred.
• Professional discipline may be occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.

Nice To Haves

• Licensure in the state where the hospital resides is preferred.
• Inpatient experience in an acute, IRF, or LTACH setting is preferred.
