About The Position

Merakey is seeking a full-time Case Manager to join their Dual Diagnosis Treatment Team in Greensburg/Indiana, PA. The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in various settings, including community living arrangements, schools, and community settings. This role involves implementing and transferring skills identified in the recovery treatment and support plan using a person-centered approach, in collaboration with the treatment team.

Requirements

Bachelor’s degree in social work, Psychology, or another human service-related field required.
Valid PA driver’s license with a clean driving record and reliable transportation required.
Experience in the behavioral health field working with individuals with mental health/developmental disabilities.

Responsibilities

Assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum.
Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc.
Implementing and transferring skills identified in the recovery treatment and support plan that utilizes a person-centered approach, in collaboration with other members of the treatment team.
