About The Position

UMOM New Day Centers is seeking a Case Manager to engage clients, build rapport, and provide direct case management support and advocacy. The role focuses on skill-building, resource connection, and employment to help clients identify, obtain, and maintain permanent housing. The Case Manager serves as the primary point of contact for community providers, ensuring collaboration across service sectors.

Requirements

  • High School or GED Diploma
  • Minimum 3 years of experience working with at-risk populations and/or case management
  • Basic computer literacy required, including data entry, web navigation, typing and text editing, and the ability to learn operations in new software systems
  • Proficiency with Windows operating system, Microsoft Word, Excel, and Outlook
  • Valid Level One Fingerprint Clearance Card or the ability to obtain one
  • Ability to clear the Central Registry Screening from the Department of Child Safety & Adult Protective Services.
  • Ability to comply with and pass all required background check and drug screening requirements, including but not limited to pre-employment and random drug screening as required/requested.
  • Valid AZ Driver's License and a driving record that falls within UMOM's policy
  • 50/100 level of car insurance coverage.

  • Demonstrated knowledge of Motivational Interviewing and Trauma Informed Care
  • Knowledge of non-profit sector; experience working with underserved or disadvantaged populations
  • Knowledge of homeless services, community resources, workforce and understanding of housing providers
  • Previous experience with Efforts to Outcomes (ETO) or HMIS

Responsibilities

  • Engage clients to build rapport and provide direct case management support and advocacy.
  • Focus on skill-building, resource connection, and employment to assist clients in identifying, obtaining, and maintaining permanent housing.
  • Serve as the primary point of contact for community providers, ensuring collaboration across service sectors.
  • Assess immediate client needs and ensure immediate connection to resources.
  • Support clients with orienting them to program rules, expectations, and UMOM's full scope of services.
  • Develop relationships through empathy, understanding unique circumstances and motivations.
  • Coordinate with the Workforce Development team to develop income-based resources for clients.
  • Identify and maintain service collaborations with outside providers.
  • Participate in service coordination team meetings with internal and outside providers.
  • Maintain a friendly, accessible, approachable, and responsive attitude to client needs.
  • Enter timely and accurate case notes and assessments per agency expectations, maintaining client confidentiality and following program guidelines.
  • Utilize evidence-based practices, including Motivational Interviewing and Trauma Informed Care.
  • Support clients in securing identification and documentation needed for housing access.
  • Thoroughly assess client needs and identify/strategize around barriers to long-term success.
  • Assist clients in co-creating a case plan for permanent housing based on unique needs and circumstances, meeting regularly to discuss progress.
  • Meet regularly and consistently with clients for structured, intentional conversations focused on skill-building for employment/income and obtaining/retaining permanent housing.
  • Transport and/or accompany clients as needed to support housing/employment search or resource connections.
  • Work in conjunction with UMOM's workforce development specialists to align employment opportunities and obtain employment for clients.
  • Ensure clients exiting to permanent housing are supported with neighborhood orientation and connections to local contacts and resources, including transportation options.
  • Perform other duties as assigned by Leadership and/or Executives.

Benefits

  • Hiring Salary Range: $46,800 - $49,800 per year
  • UMOM will cover the cost of obtaining a Fingerprint Clearance Card if the selected candidate does not currently possess one.
