The Licensed Programs Case Manager is responsible for coordinating and managing person-centered services for assigned Oparc participants through the development, implementation, and monitoring of Person-Centered Plans (PCP) and Individual Service Plans (ISP). In Licensed Programs, this role facilitates ongoing assessment and progress monitoring, and maintenance of accurate and compliant records in accordance with Developmental Disabilities Services (DDS), Community Care Licensing (CCL), and Regional Center standards. Deliver personalized, high-quality services through regular participant engagement and close interdisciplinary collaboration with families, care providers, and Regional Center partners. Provide advocacy, guidance, resource coordination, solution-focused support to remove barriers, and promote the highest level of skill development to help participants achieve greater success, independence, and alignment with their personal goals and preferences.
