About The Position

Rite of Passage Team is hiring for a Case Manager at Palm Beach Youth Academy in West Palm Beach, Florida. The academy provides a healing environment for at-risk males, offering evidence-based, mental health, and substance abuse services. The role involves administering, developing, and facilitating case plans and treatment programs, collaborating with professional staff, providing individual counseling, communicating with families and agencies, monitoring and documenting clinical behaviors, and participating in youth activities. Requirements

Possess a BA/BS in a related field.
Have prior case management experience, including three years' experience with juveniles.
Be at least 21 years of age.
Be able to pass a criminal background check, drug screen (THC not tested for pre-employment), physical, and TB test.
Be able to pass a search of the child abuse central registry.

Responsibilities

Administering, developing, and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load.
Collaborating with other professional staff to establish and meet the goals of each youth's treatment plan.
Providing individual counseling and skill development.
Communicating with families, probation officers, referral agencies, etc., on the progress of each youth on your case load.
Providing an atmosphere that is supportive of the youth’s needs.
Monitoring and documenting clinical behaviors and activities.
Participating and assisting with educational, social, athletic, and recreational activities.
