About The Position

The Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. The role monitors care through assessments and/or patient records to ensure patients are progressing toward desired outcomes, identifies and resolves barriers to effective care, and actively participates in discharge planning. The Case Manager integrates company values into daily practice. This is a PRN position working Monday through Friday day shifts as needed; weekends are not required. The role may require work during inclement weather and other staffing emergencies.

Requirements

Current state clinical licensure.
Two years of recent relevant experience.
Valid and acceptable driving record, current driver's license, and insurability.

Nice To Haves

Experience in an acute care setting.
Bachelor's degree.

Responsibilities

Communicate, collaborate, and coordinate with team members to provide quality patient care and support positive patient outcomes.
Facilitate communication during interdisciplinary team conferences.
Communicate appropriate information to payors for timely updates and authorizations.
Meet accreditation standards and adhere to requirements established by the state, The Joint Commission (TJC), and CMS.
Document communications, care coordination, patient activities, medical necessity, and post-discharge equipment and support needs in the health care record.
Support and facilitate length-of-stay management.
Conduct utilization reviews.
Promote a safe environment for patients, visitors, and staff by assessing risks and implementing appropriate precautions.
Comply with approved safety and infection prevention standards.
Develop plans of care with physicians and multidisciplinary team members, monitor patient progress, identify and resolve barriers to care, and participate in discharge planning.
