About The Position

The Womens Transitional Living Center Case Manager provides education and awareness information regarding meth, opioids and other drugs and alcohol to WTLC residents. They also provide sobriety group/meetings for Women’s Transitional Living Center residents, and appropriate documentation of services as indicated in regulation and policy. This role involves reading staff communication records, maintaining familiarity with resident records, and providing services as indicated on resident treatment and service plans. The Case Manager observes and collects in-house urinalysis and refers residents to further testing as needed. They demonstrate knowledge of and follow infection control procedures, observe changes in resident status, needs, or preferences, and communicate them appropriately. Recording resident information as required, demonstrating knowledge of and adherence to procedures for fire, life safety, disaster, security, work, safety, and other emergency procedures are also key functions. The role includes assisting with individual resident and group social recreational activities, which may involve supportive listening and skill development. The Case Manager provides back-up to other Behavioral Health Staff and transports individuals in a company vehicle for the purpose of accessing resources in support of transition back into the community, such as housing, employment, education, medical/dental care, recovery, and cultural activities. Other duties as assigned are also part of this role.

Requirements

  • Requires a High School Diploma or GED.
  • Certified Recovery Mentor or Peer Support Specialist by the State of Oregon or ability to obtain certification within six(6) months of hire.
  • One of the following required in order to meet the definition of Peer as defined by the State Of Oregon Administrative Rule: Self-identified person currently or formerly receiving mental health services, Self-identified person in recovery from a substance abuse disorder, who meet the abstinence requirements for recovering staff in alcohol and other drug treatment programs, Family member of an individual who is a current or former recipient of addiction or mental health services.
  • Experience with issues involving addiction required.
  • If a recovering chemically dependent person must be in a program of recovery and able to provide documentation of sobriety for a minimum of two (2) years OR equivalent combination of training and experience.
  • Must maintain a valid Oregon driver’s license or obtain one upon hire, qualify for the Tribe’s vehicle insurance, and provide proof of personal vehicle insurance.
  • Certified Peer Recovery Mentor.
  • Must have knowledge of the effects of trauma, coping and other aspects of the lives of those served.
  • Adheres to the organization’s policies and procedures covering the privacy, access and client health information in compliance with Federal and State laws and the healthcare organizations information privacy practices.
  • Must have a working knowledge of the Privacy Act/HIPAA and its applications.
  • Must have strong interpersonal skills working with individuals from diverse backgrounds.
  • Must respect all other regardless of age, gender, ethnicity or income level.
  • Proven ability to work independently and interpret policies and procedures, under pressure with little or no supervision.
  • Ability to work with all levels of individuals, including disgruntled people in an effective manner.
  • Must provide medical history of vaccinations.
  • Must meet in-service training hours required for job classification and position.
  • Requires attendance at mandatory in-service training.
  • Must have and maintain CPR/First Aid Certification or obtain within sixty-(60) days of hire.
  • Must be free of non-treated communicable disease.
  • Ability to work safely and to recognize potentially dangerous situations.
  • Ability/willingness to perform shift work which may include residing and sleeping the night at the facility, and to work holidays or overtime.
  • Ability to serve in an on-call capacity.
  • Ability to teach/train others.
  • Requires working knowledge and proficiency in use of office equipment and computer software such as Microsoft products.
  • Requires excellent verbal and written communication skills, business and telephone etiquette.
  • Must have the ability to work at times with minimal supervision.
  • Must have excellent organizational skills, attention to detail and record keeping skills.
  • Must have ability to respond courteously and tactfully to complaints and client requests, and to maintain confidentiality.
  • Knowledge of and sensitivity to Native American culture required.
  • Candidates for this position will be subject to criminal and character background checks and may be subject to fingerprinting.
