About The Position

The Case Manager at America’s Rehab Campus (ARC) provides comprehensive case management and coordination of care for clients in residential, detox, and/or outpatient programs. The Case Manager works collaboratively with the multidisciplinary team to ensure continuity of care, advocacy, discharge planning, and linkage to community resources in alignment with individual treatment plans and recovery goals.

Requirements

  • High School Diploma or Equivalent required
  • Minimum two (2) years of experience in related field preferred
  • CPR/FA Certification – to be obtained within first 30 days
  • Fingerprint Clearance Card – have current card or show proof of applying within 7 days from hire date
  • Handle with Care – to be obtained within first 30 days
  • Successful clearance of pre-employment criminal/county/state background check
  • Successful clearance of pre-employment drug and alcohol testing
  • Successful clearance of TB PPD testing upon hire and annual screening of TB symptoms
  • Attending clinical oversight with BHP once every 2 weeks worked for a minimum of 2 sessions per month.

Responsibilities

  • Develops, implements, and monitors individualized case management and service plans in collaboration with clients and treatment teams.
  • Coordinates all aspects of care including medical, psychiatric, social, housing, employment, and aftercare services.
  • Maintains frequent and meaningful contact with clients to ensure progress toward treatment and discharge goals.
  • Conducts assessments of client needs and barriers to treatment, documenting findings clearly and promptly in the electronic health record (EHR).
  • Collaborates with therapists, nurses, behavioral health technicians, peer supports, and physicians to ensure integrated care delivery.
  • Facilitates discharge planning, ensuring clients have appropriate referrals for continued treatment, housing, and support services.
  • Ensures accurate and timely documentation of all services, contacts, and case notes per agency, AHCCCS, and Joint Commission standards.
  • Communicates effectively with external partners such as probation officers, courts, family members, and community agencies, ensuring client consent and HIPAA compliance.
  • Assists clients in obtaining identification documents, benefits, employment, and community support resources.
  • Participates in multidisciplinary team meetings and clinical staffing discussions to advocate for client needs.
  • Provides crisis intervention and coordination of emergency services when necessary.
  • Tracks and reports on client progress, discharge outcomes, and service utilization.
  • Maintains professional boundaries and always adheres to ethical and confidentiality standards.
