About The Position

This position will work as part of a team that utilizes a Harm Reduction and Housing First Model to assist people by meeting them where they are, with housing as a primary focus. Team members will respond to each participant’s unique needs and goals by providing on-going support, advocacy, and assistance in accessing community services to ensure stability in housing and integration in the community. Case Managers must be knowledgeable about and able to respond effectively to the complexities of participants’ lives and the co-occurring challenges and conditions associated with long-term homelessness.

Requirements

  • Bachelor’s degree in human services or equal experience in the field.
  • Experience with mental health and chemical health issues.
  • Driver’s license and car required.

Nice To Haves

  • Experience working with individuals who are homeless or have personally experienced homelessness.
  • Broad knowledge of housing resources.
  • Bilingual skills are preferred.

Responsibilities

  • Partner with participants for housing stability, including addressing barriers to housing, completing tenancy education, obtaining household items, addressing housekeeping needs, and preparing for independent housing.
  • Provide effective case management to a caseload of a minimum of 18 participants, including completing intakes, assisting with applications, meeting weekly with participants (including monthly home visits), developing self-defined goals, orienting participants to program policies, offering regular contact for trust-building and crisis prevention, connecting participants with community resources, building relationships with property managers, and providing transportation to appointments.
  • Partner with program participants to increase financial stability by assisting with government benefits eligibility, connecting them with education/training/employment opportunities, and helping them develop budgets and spending plans.
  • Complete accurate and thorough documentation and recordkeeping in a timely manner, including maintaining electronic ClientTrack files with detailed case notes, understanding documentation for Housing Support, and maintaining records for timesheets, credit cards, and mileage reimbursement.
  • Provide positive representation of SHS through public relations and networking, including communicating and coordinating services with team members, attending meetings, interpreting the agency’s mission, and attending professional trainings.
  • Commitment to racial equity and social justice by working within and across agency departments to build a culture of equity and antiracism, evolving knowledge and strategy in this area, and participating in related meetings and training.
  • Demonstrate essential knowledge, abilities, and skills including initiative, critical thinking, flexibility, solution-focus, independent work, teamwork, collaboration, establishing and maintaining relationships, superior computer skills (Office 365, Word, Excel, Outlook), ability to learn new computer skills, maintaining confidentiality, promoting a professional and affirming work environment, and maintaining professional boundaries.
  • Provide back up for other Case Managers during periods of absence or increased workloads.
