About The Position
The Case Manager coordinates care across an interdisciplinary team for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether admission is at the appropriate level of care. The Case Manager identifies discharge needs, coordinates suitable discharge plans, and works collaboratively with the interdisciplinary team to help achieve desired financial and quality outcomes. The role also maintains and enhances relationships with payors.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience.
Nice To Haves
- Licensure in the state where the hospital resides is preferred.
- Experience in an acute, inpatient rehabilitation (IRF), or long-term acute care hospital (LTACH) setting is preferred.
Responsibilities
- Coordinate interdisciplinary care for a designated patient population.
- Review inpatient admission records to assess utilization of hospital resources and appropriateness of the admission level of care.
- Assess and identify discharge needs and coordinate an appropriate discharge plan.
- Collaborate with the interdisciplinary team to facilitate desired financial and quality outcomes.
- Maintain and enhance payor relationships.
