Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. No supervisory function. Requirements

  • Bachelor's Degree preferred from an accredited college or university, in Social Work or a clinical related mental health field
  • Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred
  • May be required to work flexible hours and overtime.

Responsibilities

  • Responsible for discharge plans including scheduling of appointments and communication with parents and significant family contacts
  • Assist with chart audits and completion of medical records
  • Document findings, discharge plans, and actions taken on medical record according to departmental guidelines; prepare reports and maintain records as required
  • Participate in multidisciplinary team meeting regarding the planning and implementation of patient care; facilitate communication and problem solving related to discharge planning
  • Perform a variety of clerical activities associated with the discharge planning procedures and the transfer and replacement of residents into facilities and community services agencies such as mental health programs, substance abuse programs, and outpatient programs
