About The Position

The Case Manager provides comprehensive wrap-around support services for veterans, active-duty service members, and their families receiving care at the Steven A. Cohen Military Family Clinic. This position serves as a key point of contact for clients by assisting with intake coordination, referral screening, care navigation, resource linkage, outreach activities, and collaboration with community and military stakeholders. The role supports continuity of care and helps ensure clients receive timely access to appropriate behavioral health and supportive services.

Requirements

  • Bachelor’s or Master’s Degree in Social Work, Sociology, Psychology or related field.
  • 1+ years case management experience
  • 2+ years in a customer service focused environment.
  • Driver’s License with clear record required.
  • Must have daily use of a vehicle without prior notice.
  • Must be available and willing to travel to various locations and with such frequency as the business need dictates.
  • Must be available and willing to work nights, weekends and holidays as required to meet business needs.
  • Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.

Nice To Haves

  • 3+ years preferred case management experience
  • Experience with homeless, low income, veterans & their families a strong plus.
  • LMSW preferred.
  • Bilingual preferred.

Responsibilities

  • Screen referrals for eligibility and appropriateness of clinic services according to program guidelines.
  • Provide case management and wrap-around support services to clients and families.
  • Connect clients with community resources including housing, employment, financial assistance, transportation, benefits, medical care, and social services as appropriate.
  • Coordinate care with internal clinical staff and external providers to support treatment planning and continuity of care.
  • Participate in outreach efforts, community events, and partnership development activities to increase awareness of clinic services.
  • Document client interactions, referrals, and case activities accurately and timely in accordance with clinic policies and confidentiality requirements.
  • Maintain compliance with HIPAA, ethical standards, and organizational policies.
  • Collaborate with multi-disciplinary team to perform risk management and crisis intervention services as necessary.
  • Be active in and network at monthly Veteran community groups.
  • Provide presentations at various community groups within the greater metropolitan and surrounding area.
  • Other duties as assigned.
