Lead person-centered planning (PCSP). Navigate services like Day and Residential programs, Medicaid/MCOs, SSI/SSDI, SNAP, VR, mental health & medical support. Advocate for choice, rights, and independence. Coordinate services and build partnerships in the community. Balance meaningful relationships with important documentation and case management tasks. Requirements
- Bachelor’s degree and 6+ months experience with IDD/Intellectual and Developmental Disabilities OR equivalent experience in the field.
- Valid driver’s license and good driving record.
- Ability to pass a background check and drug screening.
