About The Position

Under the direction of the Program Supervisor, we are seeking a full-time, qualified Case Manager to provide rapid rehousing to eligible individuals who are justice-involved, as well as individuals and families, through the county’s homeless Continuum of Care (CoC) and Coordinated Entry System (CES), in compliance with CoC Program Models and Performance Standards. Rapid Rehousing Case Manager helps homeless individuals and families find and maintain stable housing. The Case Manager will connect clients with resources, develop housing plans, and provide ongoing support to ensure long-term housing stability. This role often involves close collaboration with other social service agencies and landlords. The ability to maintain a thorough understanding of the CalAIM program and leverage cross-sector partnerships to support positive housing and health outcomes for participants.

Requirements

  • A minimum of 1 year working with justice-impacted and / or homeless individuals as a Case Manager
  • Self-starter, ability to work with minimal supervision, and experience working with multiple barriers.
  • Good written and oral communication skills. Good organizational skills, great attention to detail, and the ability to multitask.
  • Good analytic skills, critical thinking skills, ability to create accurate and insightful data outcomes and evaluation.
  • Strong Excel, Word, and PowerPoint presentation.
  • Proven track record of inability to manage and ability to work independently.
  • Ability to work late or on weekends as needed for activities related to the program.
  • In compliance with federal law, all persons hired will be required to verify identity and eligibility to work in the United States and to complete the required employment eligibility verification form upon hire; must pass a background check.

Nice To Haves

  • A Bachelor’s degree or equivalent in a related field is preferred.
  • Bilingual is preferred.

Responsibilities

  • Conduct comprehensive assessments to identify client needs, strengths, and barriers to housing.
  • Develop individualized housing plans that outline goals, timelines, and necessary support services.
  • Assist clients in locating and securing suitable housing, including identifying available units, completing applications, and navigating lease agreements.
  • Connect clients with relevant community resources, including social services, employment assistance, mental health and substance abuse treatment, and healthcare.
  • Maintain accurate and up-to-date client records, track progress, and submit required reports to funders and other stakeholders.
  • Administer financial assistance for rent, utilities, and move-in costs, and help clients develop and manage budgets to maintain housing stability.
  • Collect and document participant data and services delivered into the County’s Homeless Management Information System (HMIS).
  • Maintain all program-related documents for monitoring and compliance with federal, state, and local regulations under the project’s requirements.
  • Represent Lao Family Community Development (LFCD) at local collaborative community meetings, attend all the required training, employment-related meetings, or conferences as assigned.
  • Analyze and evaluate information and situations, identify key issues, consider alternatives, and make logical decisions based on sound reasoning that will achieve the desired outcome.
  • Ability to collaborate with the Contra Costa County Probation Department.
