The Community & Behavioral Services (CBS) Case Manager, non-licensed provides comprehensive, culturally responsive case management services to Tribal citizens and other participants receiving services through the CBS Healing Center. The position serves as a central point of coordination among participants, families, Healing Center providers, Tlingit & Haida departments and programs, community organizations, and external service providers. The Case Manager partners with participants to identify needs and work toward the goals established in their person-centered service or treatment plans. Depending on the participant’s needs, this may include conducting additional needs assessments, developing and coordinating individualized service plans, or supporting implementation of clinician-developed treatment plans. Responsibilities include connecting participants with appropriate resources, facilitating referrals, advocating for access to services, monitoring progress, maintaining accurate and confidential records, and coordinating communication across providers and systems. The Case Manager helps participants navigate behavioral health, medical, housing, education, employment, transportation, family, social service, and other systems that may affect their wellness, stability, and ability to achieve their identified goals. Services are provided using strengths-based, trauma-informed, culturally responsive, and participant-centered practices that honor the importance of Tribal identity, family, community, culture, traditional practices, and self-determination in healing and wellness.
