Requirements

  • Bachelor’s degree from an accredited college or university in any of the social sciences, criminal justice, or human services related field.
  • 2 years case management experience
  • Valid local driver’s license, and at least 21 years of age.

Nice To Haves

  • Certified Behavioral Health Case Manager (CBHCM) preferred

Responsibilities

  • Working cooperatively with and monitoring service delivery of a variety of community agencies and organizations to ensure maximum utilization of available resources and services to clients, in order to facilitate client recovery and enhance their lifestyle and functioning.
  • Refering clients for services, and, when necessary, advocating to the services and quality of services to which the person is entitled.
  • Serving as a resource to treatment team in identifying and resolving problems, and providing appropriate alternative sources for services and support required by clients; participating in interdisciplinary meetings to review treatment progress, identify strengths and needs related to the development of, to review and update the individual case management plan, and relay and exchange relevant information concerning problems affecting the clients’ response to treatment.
  • May develop or assist in the development of a safety plan for clients with suicidal ideation or locate and coordinate services and resources to resolve a client crisis.
  • Explaining policies and regulations, and orienting clients to treatment program and facility.
  • Performing case monitoring which includes contacting clients who have unexcused absences from the program in order to re-engage, reporting program attendance and participation to probation or parole officers, caseworkers, and other court appointed officials; completing scheduled appointment follow-up, developing discharge plans, and ensuring resources are consistent with overall treatment objectives.
  • Preparing daily and monthly reports regarding clients’ treatment progress.
  • Maintaining communication with supervisor and service providers in order to keep informed of potential sources of assistance in resolving casework problems, identify and respond to gaps in client services, coordinate provision and utilization of services and provide follow up.
  • Gathering data to determine clients’ treatment outcome by conducting discharge surveys.
  • Recording relevant information and summarizes data according to established procedures.
