About The Position

Riverbend Community Mental Health is seeking compassionate, enthusiastic, and driven Case Managers to join their Team as part of the Community Support Program. This role involves working alongside a dynamic team of professionals dedicated to changing lives through integrated, person-centered care. The team specializes in supporting adults with complex needs, including those with severe mental illness and co-occurring intellectual or developmental disabilities (MI/ID).   • A valid driver’s license, personal vehicle, and auto insurance with minimum coverage of 100,00/300,00 are essential.
  • Travel throughout Riverbend’s catchment area to various towns, community locations, client homes, state agencies, or other locations necessitated by job duties is required.
  • Previous experience in mental health more specifically with severe and persistent mental illness, intellectual and/or developmental disabilities.

  • Provide well-integrated case management that helps clients navigate their world and achieve their personal goals.
  • Work with a diverse clientele, including those with dual diagnoses and adults through all stages of life.
  • Partner with a dedicated, multidisciplinary team to ensure every individual receives the holistic support they deserve.
