HealthRIGHT 360 is a nonprofit organization dedicated to providing accessible healthcare to vulnerable populations, aiming to tackle systemic barriers and promote health equity. Prototypes Women's Center in Pomona is a 180-bed residential facility for women, offering a supportive environment for recovery from co-occurring disorders. The program integrates physical and behavioral health, addressing social determinants of health, resilience, and recovery. Children can be involved in the recovery process, accessing mental health support and educational assistance. The interdisciplinary team provides comprehensive treatment for substance use disorders, mental health, parenting, education, and life skills. Treatment duration ranges from 60 days to 6 months. The Case Manager provides in-person support, coordinating care within HealthRIGHT 360 and with external providers. Responsibilities include completing intake paperwork, conducting assessments, and assisting with benefit applications. Services are delivered flexibly in outpatient offices, residential facilities, or the community, requiring reliable transportation.
