About The Position

As a Case Manager, you'll play a crucial role in preparing residents for their successful reentry into the community. You'll provide support and guidance, ensuring that their criminogenic risks, needs, and responsivity are addressed through individualized program plans. From conducting initial risk assessments and developing rapport with residents to providing thorough program orientations and ensuring compliance, your role is dynamic and impactful. You'll assist residents in creating effective release plans, conduct random room visits, and perform weekly medication audits. Accurate documentation and active participation in quality assurance and training activities are also key aspects of your job. Additionally, you'll use cognitive behavioral tools to deliver evidence-based interventions, reinforce positive behavior, and model pro-social behavior. Reporting significant developments to your supervisors promptly is essential to maintaining the integrity and effectiveness of the program. The Passages Pre-Release team is led by an experienced Program Supervisor who has a passion for the work we do and supports their team to be the best they can every day, for our residents. Requirements

  • Bachelor’s degree in human services or a related field, or at least 5 years of experience in corrections, case management, or a similar field
  • Ability to relate to residents in a firm yet empathetic manner
  • Computer and word-processing skills, with demonstrated experience in documentation and record keeping
  • Valid state driver’s license
  • Ability to pass federal drug screening requirements and a background check
