Requirements

  • Registered Nurse with New York State Licensure or Master’s Degree in Social Work (MSW) from an accredited School of Social Work with Licensure to practice in NYS or eligibility for licensure with valid permit is required.
  • Demonstrated assessment skills.
  • Ability to utilize problem solving theories and techniques, combined with flexibility and creativity, in working with a diverse population.
  • Must possess time management skills, highly developed verbal and written skills, problem solving capacity, critical thinking, and good organizational skills.
  • Requires knowledge of community resources, third party payers, and regulatory agencies.
  • Position will primarily be light work requiring lifting up to 25 pounds with occasional lifting and/or carrying objects weighing up to 25 pounds.
  • Occasional bending, twisting, stooping, kneeling and reaching is required.
  • Requires 60% sitting and 40% standing or walking.
  • Expressing or exchanging ideas by spoken word is required.
  • Works in a clean, well-lighted, heated or ventilated facility.
  • No routine exposure to hazards.
  • May have occasional exposure to blood, body substances, infectious materials, chemical hazards, vibration, radiation, slippery surfaces, or noise that may result from performance of duties.
  • Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks.
  • Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment.
  • Can withstand long periods of sitting, standing and/or constant walking.
  • Ability to lift 25 lbs.
  • Ability to foster collaborative relationships, to work well under pressure, to organize and synthesize new information, and prioritize tasks.
  • Possesses critical thinking, analytical skills and flexibility.
  • Ability to multi-task.
  • Required detailed attention to work in an environment where interruptions cannot be controlled.
  • Demonstrates sensitivity to customer needs and expectations.

Nice To Haves

  • Licensure is expected within 12 months of employment.
  • Competency on computer is preferred.
  • Case Management Certification within 36 months of employment is preferred.
  • May be subject to irregular hours including evenings or potentially weekends to participate in operational and community events as necessary.

Responsibilities

  • Coordinates the multi-disciplinary activities related to maintaining continuity of care from the hospital to home setting.
  • Responsible for discharge planning, utilization review, team conferences and unit based rounds.
