About The Position

Nestled in the Sierra foothills of California, Yosemite Treatment Center located on the Sierra Ridge Campus is a licensed Short-Term Residential Therapeutic Program that offers individual, group, and family therapy, as well as evidence-based skill and rehabilitation groups. Yosemite Treatment Center embraces a strengths-based approach within a least restrictive program model and normalized high school environment. Here youth become part of a cohesive, supportive, dynamic community of peers and highly qualified staff. Through Rite of Passage’s Integrated Care Model, Yosemite Treatment Center provides a trauma-responsive program that is individualized for each youth.

As a Case Manager, you will have the unique opportunity to create a positive, safe and supportive environment for the youth we serve while building a career rich in growth opportunities and self-fulfillment.

Requirements

• Be empathetic, patient, consistent, compassionate, an honest work ethic, and treat others with respect
• Have a desire to work with troubled teens
• Have a BA/BS in a related field
• Have prior case management experience, including 3 years with youth and/or juvenile placement agencies
• Be at least 21 years of age
• Be able to pass a criminal background check, drug screen, physical, and TB test
• Be able to pass a search of the child abuse central registry.

Responsibilities

• Administrating, developing and facilitating the completion of case plans and treatment programs
• Develop and oversee the completion of the MAP Plan
• Provide monthly progress notes and weekly student updates
• Facilitate weekly group meetings
• Conduct assigned responsibilities with respect, consideration, encouragement, positive guidance, and utilization of resources towards the youths’ overall success.
