LFCD is seeking a qualified and motivated Case Manager to support justice-involved individuals in obtaining and retaining meaningful employment as well as manage participants under the California Advancing and Innovating Medical (Cal-AIM) Initiative, which provides comprehensive care management to address both clinical and non-clinical needs. This position serves as the primary point of contact for participants and is responsible for developing relationships with local employers, training providers, community-based organizations, and internal LFCD departments, particularly working with Workforce Innovation Opportunity Act (WIOA) program staff and America’s Job Center of California (AJCC) Business Centers. The Case Manager will recruit and enroll justice-involved individuals, connect participants with fair-chance employers, provide employment readiness services, career coaching, and comprehensive case management support. This position requires travel and rotating assignments among multiple office locations as needed. The Case Manager reports directly to the Program Supervisor.
