About The Position

The Case Manager coordinates the implementation of Individual Employment Plans (IEPs). This role assesses and monitors the implementation and effectiveness of clinical interventions, conducts individual and group therapeutic sessions with program participants, and coordinates behavior management. The Case Manager discusses areas affecting program participants with related health services, DDD case managers, DVR counselors, Support Coordinators, and families/legal guardians/group home staff. This role fosters input in communications, is aware of HIPAA regarding Personal Health Information (PHI) and recognizes that the highest quality of service is delivered with Hospitality, Respect, Quality, Spirituality, and Responsibility, the core values of the Agency, and maintains confidentiality of departmental records, persons, and activities. Requirements

  • B.S. in Psychology, VR or related Human services field.
  • One (1) to three (3) years prior adult counseling or related work with persons with disabilities.
  • Valid driver’s license with appropriate levels of insurance; CDL or eligibility.
  • Eligible for C.R.C. and willingness to pursue.
  • Ability to facilitate and coordinate the work of others.
  • Knowledge of non-profit educational/training environment.
  • Familiarity with funders such as DVRS and DDD.
  • Required skills, knowledge, and ability to perform job.
  • Ability to develop and maintain effective work relationships.
  • Ability to learn new tasks quickly.
  • Effective oral and written communication skills.
  • Close vision: Clear vision at 20 inches or less.
  • Distance vision: Clear vision at 20 feet or more.
  • Color Vision: The ability to identify and distinguish colors.
  • Peripheral vision: To observe an area that can be seen up/down or to the left/right, while the eyes are fixed on a given point.
  • Depth perception: The ability to judge distance and spatial relationships.
  • Focus adjustment/ability: To adjust eyes to bring an object into sharp focus.
  • Hear/process for conversation speech.
  • Effective articulation.

  • Read and write at college level.

  • M.S. preferred with upper experience range.
  • Already obtained CRC preferred.
  • ASL or Deaf Studies preferred.
