The Child and Family Case Manager is responsible for coordinating person-centered planning, attending to the total spectrum of the individual's needs, including housing, family relationships, social activities, education, finance, employment, health, recreation, mobility, protective services, and records. This role involves collaborating with consumers and staff to assess needs, develop treatment goals, objectives, methodologies, and timelines, and evaluate progress. The Case Manager will locate, obtain, and coordinate services both within and outside the CMH system, provide supportive services to consumers and their families, and offer crisis intervention when appropriate. They will also facilitate interdisciplinary team evaluations, conduct assessments, monitor progress, and maintain liaison with outside providers. Additionally, the role involves participating in quality assurance, maintaining knowledge of mental health strategies, monitoring treatment plan follow-up, facilitating consumer benefits, and coordinating decisions regarding hospitalization and level of care changes. The Case Manager may also be involved in medication delivery and observation.
