JOIN OUR TEAM AND MAKE A DIFFERENCE! Primary Job Purpose: Assuring residents psychosocial needs are met through the care plan process. Essential Job Functions: Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center. Shall provide orientation to new residents and their families (including explanation of residents rights, advanced directives, house rules, etc.). Participate in the admission process, including assessments of potential residents as required. Addressing psychosocial needs of individuals or groups, either directly or by referral to community agencies as needed. Assist residents and families in coping with the medical and psychosocial aspects of residents illness and disability as well as the stay in the care center. Communicate with residents families when concerns or needs arise. Provide appropriate referrals to community services such as mental health. Coordinate transfers (room changes) within the care center and assist resident in adjusting to the transfer. Facilitate resident groups as needed. Assist resident with accessing outside resources, such as applying for Medicaid, when family is not available. Assist resident with financial management. Respond to resident complaints and act according to Eben Ezer’s grievance procedure. Respond to allegations of abuse/neglect according to the Eben Ezer policy. Shall review pertinent resident incident reports and respond to issues of immediate concern or trends. Participate in resident assessment (MDS) and care planning, reviewing and updating at least quarterly or as scheduled. Develop a discharge plan for each resident and record in the medical record within fourteen days of admission. Update quarterly in progress notes. Record information of social history in the medical record within fourteen days of admission and review at least annually. Participate in activities as needed, attend lunch club and outings. Record progress notes in the medical record at least quarterly, the initial progress note being done within fourteen days of admission. Attend required meetings such as Care Conferences, Psychotropic drug review, Mental Health Services review and others as needed. Participate in developing policies and procedures pertaining to the social services department. Provide required education to Eben Ezer staff for new employee orientation, CNA training and annual in-services as needed. Obtaining the necessary knowledge of State and Federal Regulations. Assist with neighborhood meetings, resident council, other activities and other large community-wide events, as needed. Must maintain confidentiality of all work related information according to HIPAA regulations. Assure residents rights, privacy, and HIPAA regulations are followed. Must be able to push/pull up to 250 pounds regularly using a wheelchair. Must be able to lift 0-50 pounds regularly by self and 50+ pounds occasionally with assistance. In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.
