Under the supervision of the Case Management Program Manager, the Case Manager will be responsible for providing ongoing engagement, assessment, intervention, and evaluation for the youth on their individual case load. This commonly includes youth intakes, assessments, crisis phone counseling, client data collection, documentation, file maintenance, and follow-up care. In addition, the Case Manager will assist with the general upkeep of the case management program, including regular program shift coverage, milieu management, crisis intervention, reporting maintenance issues, and ensuring that the program is facilitated in a manner that meets Urban Peak’s mission. Case Managers will work with youth ages 12-24 who are enrolled in Urban Peak’s programming who are experiencing homelessness. The acuity of clients varies, but many clients will have unique goals related to mental health, substance use, education, employment, and housing. This position will ensure that interactions, interventions, and environments at Urban Peak are trauma informed and will emphasize the physical and emotional safety of youth and staff.
