About The Position

SSM Health Rehabilitation Hospital, a joint venture between Select Medical and SSM Health, is seeking a full-time Case Manager for their inpatient rehabilitation hospital located in Bridgeton, MO. This role focuses on providing exceptional and compassionate care to individuals with a variety of complex medical conditions, including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputations, and multiple traumas. Requirements

• Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.

Responsibilities

• Coordinate health care decisions using a systematic approach to improve quality and outcomes.
• Coordinate care across the continuum.
• Promote cost-effective care within the allotted time frame.
• Assure payments of hospital-based services meet patient-related utilization management criteria.
• Implement safe and appropriate discharge plans.
• Assess the psychosocial needs of the patient and provide intervention as part of the discharge planning process.
