About The Position

Case Managers will assist with the execution of South Georgia Healthy Start's daily activities to support women, infants, and families in rural South Georgia. Requirements

  • A bachelor's degree from an accredited college/university.
  • Two or more years relevant experience, which could include case management, patient navigation, health education, community engagement, survey research, interventionist work or doula/birthworker experience.
  • A valid driver's license.
  • Must be insurable by the university's carrier as this position requires travel.
  • Microsoft Office proficiency.
  • Exceptional interpersonal skills.
  • Strong verbal and written communication skills.

Responsibilities

  • Assist with the execution of South Georgia Healthy Start's daily activities to support women, infants, and families in rural South Georgia.
  • Recruit, enroll, assess, and retain participants.
  • Provide care navigation and social services enrollment assistance.
  • Perform routine check-ins with participants.
  • Deliver evidence-based curricula and health education.
  • Conduct parenting education classes.
  • Actively engage community partners within assigned service region.
  • Participate in professional development and trainings as applicable.
  • May be assigned to lead committees or initiatives within the South Georgia Healthy Start program.
