About The Position

Transitional Services for New York, Inc. (TSINY) is a comprehensive, community-based mental health organization in New York City, providing rehabilitative services to individuals recovering from mental illness. Their mission is to enrich lives and facilitate transitions to increased independence, with a vision to broaden services and become a regional social service provider. TSINY staff are expected to deliver effective programs with compassion, integrity, and professionalism, prioritizing clients' needs and fostering hope. TSINY is seeking Case Managers for its mental health residential program in Jamaica (Queens), NY. The Case Manager's role involves coordinating and monitoring the assessment, treatment planning, and care coordination for residents on their caseload, acting as an advocate. Case Management staff work in partnership with residents to establish goals, find appropriate supports, and access independent community housing.

Requirements

  • Bachelor’s degree or Associate’s degree plus one years' experience or High School diploma plus two years’ experience in mental health setting.
  • Valid Driver License Required
  • Be able to sit or stand as needed, with or without reasonable accommodation.
  • May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation.
  • Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.

Responsibilities

  • Assist residents in developing the skills, direction and motivation necessary to graduate to more independent housing settings.
  • Assist residents to develop life skills in the direction of maximum autonomy.
  • Assist residents to link up with community resources, such as employment, day programs, vocational rehabilitation services and health care services.
  • Insure that residents receive periodic medical and dental attention.
  • Maintain contact with: (1) TSINY Financial Specialist and follow up on related benefit matters, (2) all related treatment providers to insure integrated care between the residential program and other services.
  • Meet on a regular basis with interdisciplinary team for assessment and information on resident.
  • Facilitate skill development classes.
  • Develop person-centered Service Plans.
  • Work with counselors to monitor the implementation of Service Plan goals and evaluate progress.
  • Visit hospitalized clients.
  • Monitor medication.
  • Provide crisis management when required.
  • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
  • Perform other related duties as required.
