TSINY is seeking Case Managers for its mental health residential program in Jamaica (Queens), NY. The Case Manager's role involves coordinating and monitoring the assessment, treatment planning, and care coordination for residents on their caseload, acting as an advocate. Case Management staff work in partnership with residents to establish goals, find appropriate supports, and access independent community housing.
