About The Position
Join our team as a Case Manager in a vibrant care coordination environment. The Case Manager will handle an average of 15-20 patient consultations per day in an acute care setting. The role encompasses both utilization review and discharge planning within a supportive team that values effective outcomes and patient-centered care.
Requirements
- Current BLS certification required.
- Recent experience with Dignity Health in California is essential.
- Proficient in Cerner charting system.
- Strong communication and collaboration skills.
Nice To Haves
- Training in Interqual will be provided.
Responsibilities
- Conduct initial assessments and discharge planning.
- Manage utilization review processes.
- Collaborate with various payers, including uninsured and homeless populations.
- Utilize Interqual criteria for review and approval.
- Participate in effective communication and teamwork within the case management team.
- Adapt and reprioritize tasks throughout the day based on evolving patient needs.
