About The Position
IPMG is offering a $500 sign-on bonus within the first 6 months of employment. IPMG is 100% employee-owned through an Employee Stock Ownership Plan (ESOP), meaning all employees share in the company's success. This is a full-time, hybrid position that involves remote work from a home office and travel up to one hour for community-based meetings several times a week. Candidates must reside in Indiana or within 10 miles of the state line to accommodate in-person meetings.
Requirements
- A valid driver's license, car insurance, and reliable transportation
- A Bachelor’s Degree in Human Services
- A minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities
- Designated home office setting with computer, internet service, and smart phone

- Reside within Indiana or within 10 miles of the state line
Responsibilities
- Provide information and education to help Individuals understand the Medicaid waiver process
- Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
- Help Individuals access services and supports they need to meet their goals
- Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
