Requirements

  • Bachelor’s degree in Human Services or related field preferred (Equivalent education and experience combinations considered)
  • 1–2 years of related experience working in human services, vocational training, case management, or disability services
  • Excellent communication, organization, and problem-solving skills
  • Passion for empowering individuals and promoting inclusion
  • Valid driver’s license and proof of auto insurance
  • Required Clearances
  • Ability to successfully complete: Wisconsin Caregiver Background Check, State & Federal Criminal Background Checks, Wisconsin DOT Check, Department of Justice Check, Office of Inspector General Check

Responsibilities

  • Manage a caseload of participants while ensuring services are delivered on time and according to program standards.
  • Develop individualized, outcome-based goals aligned with each participant’s Person-Centered Plan.
  • Provide hands-on job training, skill development, and workplace coaching using a strengths-based, person-centered approach.
  • Help participants build confidence, adjust to workplace routines, and develop positive work habits.
  • Serve as a key advocate by coordinating resources, services, and supports identified in participant plans.
  • Connect participants to valuable community resources and career development opportunities such as: Job Club & Job Shadows, Community Exploration, DVR Services, Community Coaching.
  • Design individualized support plans, protocols, and incentive programs that encourage attendance, accountability, and workplace success.
  • Identify and address barriers including absenteeism, peer interactions, supervision acceptance, and workplace behaviors.
  • Partner with families, employers, community agencies, and professional support teams.
  • Facilitate annual and semi-annual review meetings to evaluate progress and update goals.
  • Train staff, volunteers, interns, and production partners on participant supports and best practices.
  • Maintain accurate participant records and documentation in the Apricot Records System.
  • Prepare required reports, case notes, progress reviews, intake/discharge documentation, incident reports, and funding-related paperwork.
  • Collect program data to support quality improvement and service excellence.
  • Assist with AM/PM bus duty and participant transitions.
  • Support participants during lunch and break periods to ensure safety and engagement.
  • Administer medications to authorized participants as needed.
