About The Position

We are seeking a Case Manager who will participate as a member of a treatment team to design and implement care plans for patients with a wide range of behavioral health conditions. Assists patients in accessing healthcare services and resources to meet healthcare needs, utilizing a trauma-informed, patient centered approach focusing on patient identified goals. Assists patients in improving overall quality of life through improvement in health, relationships, meaningful activities and access to resources.

Requirements

  • Bachelor’s degree in health or human service-related field
  • Minimum of 1 year in health or human services setting
  • Current driver’s license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
  • Advanced Microsoft computer skills required.
  • Exceptional customer service for internal and external parties.
  • Excellent interpersonal skills and demonstrated ability to work effectively both independently and as part of a team.
  • High attention to detail is essential.
  • Annual Flu immunization
  • Annual TB screening
  • De-escalation training within 90 days of hire
  • BLS certification required within 90 days of hire

  • 2 years case management experience

Responsibilities
