About The Position

We are seeking a Case Manager who will participate as a member of a treatment team to design and implement care plans for patients with a wide range of behavioral health conditions. Assists patients in accessing healthcare services and resources to meet healthcare needs, utilizing a trauma-informed, patient centered approach focusing on patient identified goals. Assists patients in improving overall quality of life through improvement in health, relationships, meaningful activities and access to resources.

Requirements

Bachelor’s degree in health or human service-related field
Minimum of 1 year in health or human services setting
Current driver’s license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
Advanced Microsoft computer skills required.
Exceptional customer service for internal and external parties.
Excellent interpersonal skills and demonstrated ability to work effectively both independently and as part of a team.
High attention to detail is essential.
Annual Flu immunization
Annual TB screening
De-escalation training within 90 days of hire
BLS certification required within 90 days of hire
