About The Position

The Canadian Mental Health Association is looking for a Case Manager for the Rosemount Integration Support Program. The Rosemount program is 24/7 staffed with a dedicated multidisciplinary team that includes a Manager, Case Manager, and a Transitional Housing Support Workers. This core team will work collaboratively with external services to provide employment supports, peer support, nursing, and primary care, ensuring comprehensive and coordinated care for clients. The Case Manager will support the residents' mental health and social care needs, ensuring access to primary care and navigation of health and social care systems. The program is designed to promote housing stability, recovery, harm reduction, and community integration, while respecting tenant autonomy and dignity.

Requirements

  • An undergraduate degree or diploma in social services or related field.
  • A minimum of two years’ experience in direct service delivery.
  • Work experience in the Social Work and/or Community work sectors to adults with psychiatric disabilities.
  • Sound knowledge of crisis intervention/prevention, addiction issues, recovery-oriented approaches, culturally competent care, trauma informed care, as well as severe and persistent mental illness.
  • Ability to work flexible hours, including evenings and weekdays.
  • A solution-focused approach to work and the ability to manage crises.
  • Ability to work with diverse cultural and linguistic backgrounds/communities, demonstrating a non-homophobic and anti-racist approach.
  • Ability to work within a multidisciplinary team.
  • Ability to work independently.
  • Effective analytical, assessment, and decision-making skills.
  • Trained in trauma-informed care, family-centered practices, culturally competent care, and motivational interviewing.

Nice To Haves

  • Experience in community mental health.
  • Lived experience with refugee/settlement challenges or mental health and addiction issues.
  • Personal experience with mental health issues, either through self or a loved one.

Responsibilities

  • Provide on-site Case Management support tailored to residents' needs, including developing personalized care and relapse prevention plans.
  • Deliver mental health counseling services, including assessment, information, orientation, and housing support at the Rosemount location.
  • Facilitate internal/external referrals for services not provided by the program, including specialized and individualized supports.
  • Conduct comprehensive assessments to understand clients’ needs, strengths, and goals. Develop and implement individualized service plans in collaboration with clients and their support networks.
  • Organize, plan, and facilitate group and individual sessions for skill-building, life skills training (e.g., household maintenance, budgeting), and addressing diverse client needs.
  • Develop and implement community outreach plans to promote programs and services.
  • Collaborate with clients, programs, settlement services, and relevant parties to create individualized case management plans, including access to primary care, employment resources, and navigation through health, social care, and legal systems.
  • Support clients through individualized case management, life skills coaching, and service planning to promote successful transitions to independent living.
  • Offer transitional shelter support, develop safety and long-term housing plans, and create post-transitional support plans for ongoing housing and social care needs.
  • Conduct client assessment/individual service planning and develop therapeutic alliance with clients and their support systems.
  • Build and maintain a trusting respectful and collaborative working relationship with clients and be able to provide supportive counseling for a range of issues, skills teaching as well as psycho-education around illness, medications and symptoms.
  • Develop, implement and monitor the service plan and conduct follow-ups with the clients as needed.
  • Advocate on behalf of the client and connect them with community resources as well as establishing effective working relationships with staff of other agencies.
